Every care authority in Europe is faced with different conditions when it comes to integrated care model development, may these be of economic, technological, organisational, political or socio-cultural nature. Against this background, VIGOUR developed a context-sensitive support scheme which helps care authorities in developing the capacities and capabilities to implement good practices in integrated care.
Over the course of the project, 15 regions across seven European countries implemented local scaling-up pilots within day-to-day care settings. Based on a critical review of existing examples of successful service integration, each VIGOUR site received tailored support taking into account the different local needs, ambitions and capabilities prevailing in each region. A crucial part of this support process represents mutual learning and knowledge exchange between involved regions. VIGOUR brought together regions that have already deployed ambitious and innovative approaches to delivering person-centered integrated care (“pioneers”) with regions that weren't yet well prepared when it comes to aspects of service integration (“followers”). With the help of the experiences gained from the VIGOUR regions, VIGOUR generated evidence-based guidance material that was also made available to other European care authorities.
